Objetivo: medir el volumen urinario por medio de la ecografía vesical, realizado por una enfermera en pacientes críticos, después de la retirada de la sonda urinaria permanente y verificar los factores relacionados en la retención urinaria. Método: estudio cuantitativo, observacional y transversal, realizado con 37 pacientes críticos de ambos sexos, mayores de 18 años, con retiro de catéter vesical permanente en las últimas 48 horas. Se utilizó un cuestionario con variables sociodemográficas y clínicas y el examen ecográfico. Se utilizó un cuestionario con variables sociodemográficas y clínicas y el examen ecográfico. Los datos fueron presentados a través de distribución de frecuencias, medidas de centralidad y variabilidad, asociación mediante la prueba exacta de Fisher y, para el análisis, regresión logística binomial múltiple. Resultados: de los 37 pacientes, en su mayoría fue de sexo masculino, con una edad média de 54,9 años. La medición del volumen urinario por ecografía osciló entre 332,3 y 950 ml, y el 40,54% de los pacientes presentó retención urinaria. La retención urinaria se asoció significativamente a la aparición de infección urinaria, estreñimiento intestinal y diuresis por rebosamiento espontáneo. Los pacientes con infección del tracto urinario tenían 7,4 veces más probabilidades de tener retención urinaria. Conclusión: la ecografía vesical fue eficaz para medir el volumen urinario después de retirar el catéter urinario permanente y puede contribuir a la detección de retención urinaria.


Objective: to measure urinary volume through bladder ultrasound, performed by a nurse in critically ill patients, after removal of the indwelling urinary catheter and to verify the related factors on urinary retention. Method: quantitative, observational and cross-sectional study, carried out with 37 critically ill patients of both sexes, over 18 years of age, with removal of indwelling urinary catheter in the last 48 hours. A questionnaire containing sociodemographic and clinical variables and an ultrasound examination were used. Data were presented through frequency distribution, centrality and variability measures, association using Fisher`s exact test and, for analysis multiple binomial logistic regression analysis. Results: the 37 patients were mostly male, with a mean age of 54.9 years. The measurement of urinary volume by ultrasound ranged from 332.3 to 950 ml, and 40.54% of patients had urinary retention. Urinary retention was significantly associated with the occurrence of urinary tract infection, intestinal constipation and spontaneous overflow diuresis. Patients with urinary tract infection were 7.4 times more likely to have urinary retention. Conclusion: bladder ultrasonography was effective in measuring urinary volume after removal of the indwelling urinary catheter and and may contribute to the detection of urinary retention.


Objetivo: mensurar o volume urinário por meio da ultrassonografia de bexiga, realizada por enfermeiro em pacientes críticos, após a remoção do cateter vesical de demora, e verificar os fatores relacionados na retenção urinária. Método: estudo quantitativo, observacional e transversal, realizado com 37 pacientes críticos de ambos os sexos, idade superior a 18 anos, com retirada de cateter vesical de demora nas últimas 48 horas. Foram utilizados um questionário contendo as variáveis sociodemográficas e clinicas e o exame de ultrassonografia. Os dados foram apresentados por meio da distribuição de frequência, medidas de centralidade e de variabilidade, associação pelo teste exato de Fisher e, para análise a regressão logística binomial múltipla. Resultados: dos 37 pacientes, a maioria era do sexo masculino, com média de idade de 54,9 anos. A mensuração do volume urinário pela ultrassonografia variou de 332,3 a 950 ml, sendo que 40,54% dos pacientes apresentaram retenção urinária. A retenção urinaria apresentou associação significativa para a ocorrência de infecção do trato urinário, constipação intestinal e diurese espontânea por transbordamento. Pacientes com infecção urinária tiveram 7,4 vezes mais chance de apresentar retenção urinária. Conclusão: ultrassonografia de bexiga foi eficaz para mensurar o volume urinário após a remoção do cateter vesical de demora e poderá contribuir na detecção da retenção urinária.

OBJETIVO: Se presenta una serie de casos de reparación por vía vaginal de fístula vesicovaginal (FVV) de nuestro centro. MATERIAL Y MÉTODOS: Estudio observacional descriptivo. Se evaluaron todas las pacientes con reparación quirúrgica de FVV en el Centro de Innovación de Piso Pélvico del Hospital Sótero del Río entre 2016 y 2022. RESULTADOS: Se reportaron 16 casos, de los cuales el 81,3% fueron secundarios a cirugía ginecológica. En todos se realizó la reparación por vía vaginal, con cierre por planos. En el 94% (15/16) se logró una reparación exitosa en un primer intento. El tiempo de seguimiento poscirugía fue de 10 meses (rango: 3-29). No hubo casos de recidiva en el seguimiento. Una paciente presentó fístula de novo, la cual se reparó de manera exitosa en un segundo intento por vía vaginal. Se reportaron satisfechas con la cirugía 15 pacientes, con mejoría significativa de su calidad de vida. Una paciente reportó sentirse igual (6,3%), pero sus síntomas se debían a síndrome de vejiga hiperactiva que la paciente no lograba diferenciar de los síntomas previos a la cirugía. CONCLUSIÓN: Las FVV en los países desarrollados son secundarias a cirugía ginecológica benigna. La cirugía por vía vaginal en nuestra serie demostró una alta tasa de éxito, con mejora significativa en la calidad de vida de las pacientes.


OBJETIVE: We present a case series of vesico-vaginal fistulas (VVF) vaginal repair in our center. MATERIAL AND METHODS: Descriptive observational study. All patients with surgical repair of VVF at the Centro de Innovación en Piso Pélvico of Hospital Sótero del Río were evaluated between September 2016 and September 2022. RESULTS: 16 cases were reported. 81.3% were secondary to gynecological surgery. In all cases, a vaginal repair was performed, with a layered closure. 94% (15/16) had no contrast extravasation at the time of examination, confirming fistula closure. The follow-up time was 10 months (range: 3-29). There were no cases of recurrence during follow-up. 1 patient presented de novo fistula which was successfully repaired in a second attempt vaginally. 15/16 patients reported being satisfied with the surgery, with significant improvement in quality of life. 1 patient reported feeling the same (6.3%), but her symptoms were due to overactive bladder syndrome that the patient could not differentiate from the symptoms prior to surgery. CONCLUSION: VFV in developed countries are mainly secondary to benign gynecological surgery. Vaginal surgery in our series achieved a significant improvement in the quality of life of patients.

ABSTRACT

El presente trabajo toma como base el documento: "Manejo Urológico del Mielomeningocele" de las Guías de Atención Pediátrica, del Hospital De Pediatría "Juan P. Garrahan" de la ciudad de Buenos Aires - Argentina; de los autores: Dra. Carol Burek y Dra. Liliana Campmany. En la Unidad Técnica de Cirugía Pediátrica del Hospital de Especialidades Carlos Andrade Marín, se atienden por mes unos 50 a 70 pacientes afectos de vejiga neurogénica desde el nacimiento hasta la adolescencia. Es una enfermedad crónica que requiere un diagnóstico correcto con estudios de imagen y función de la vía urinaria además de un posterior manejo diario por parte de los padres con la guía del médico especialista.


This work is based on the document: "Urological Management of Myelomeningocele" from the Pediatric Care Guidelines of the Hospital De Pediatría "Juan P. Garrahan" of the city of Buenos Aires - Argentina; by the authors: Dr. Carol Burek and Dr. Liliana Campmany. In the Pediatric Surgery Technical Unit of the Carlos Andrade Marín Specialties Hospital, 50 to 70 patients affected by neurogenic bladder from birth to adolescence are treated every month. It is a chronic disease that requires a correct diagnosis with imaging and urinary tract function studies, as well as subsequent daily management by the parents under the guidance of the specialist.

ABSTRACT Objective: To map and to identify the material distributed in the health care network to urinary catheter users in a municipality in the state of São Paulo, Brazil. Method: Quantitative, descriptive-exploratory study, carried out with supervisors and/or managers of health services and a unit of distribution of materials used by urinary catheter users. Data were collected using a questionnaire and an interview via electronic form and analyzed by descriptive statistics. Results: Health services to urinary catheter users were mapped in five health regions: health centers, family health units, outpatient clinics, specialized centers, hospitals, and emergency care units. Intermittent catheter and indwelling catheter were the distributed materials, along with other essential materials. Conclusion: The study shows the high number of distributed urinary catheters; primary health care was the service that directed the care from the materials distribution. It is concluded that mapping the provided care allows for understanding the provided care as well as fostering future studies addressing healthcare network strategies for users of urinary catheter.


RESUMEN Objetivo: Mapear e identificar los dispositivos que disponen en la red de atención a usuarios de cateterismo urinario en una ciudad del estado de São Paulo. Método: Estudio cuantitativo, descriptivo exploratorio, realizado con supervisores y/o gestores de los servicios de salud y una unidad de distribución de materiales utilizados por los usuarios de cateterismo. Para la recolección de datos se utilizaron el cuestionario y la entrevista a través de un formulario electrónico; y para el análisis, la estadística descriptiva. Resultados: Se mapearon los servicios para usuarios de cateterismo urinario en cinco regiones de salud: unidades básicas, unidades de salud familiar, ambulatorios, clínicas especializadas, hospitales y unidades de urgencias. Los dispositivos urinarios que disponen fueron el catéter intermitente y el catéter permanente, además de otros materiales necesarios para el procedimiento. Conclusión: El estudio apunta al elevado número de catéteres urinarios distribuidos; la atención primaria de salud fue el servicio que más brindó cuidados a partir de la distribución de los materiales. Se concluye que la construcción de un mapa del cuidado posibilita identificar el cuidado ofrecido y el desarrollo de otros estudios con estrategias de cuidado en red para los usuarios de cateterismo urinario.


RESUMO Objetivo: Mapear e identificar os dispositivos dispensados na rede de atenção à saúde aos usuários de cateterismo urinário de um município do interior do Estado de São Paulo. Método: Estudo quantitativo, descritivo-exploratório, realizado com os supervisores e/ou gestores de serviços de saúde e uma unidade de distribuição de materiais utilizados por usuários de cateterismo. Os dados foram coletados utilizando-se questionário e entrevista via formulário eletrônico e analisados por estatística descritiva. Resultados: Foram mapeados os serviços aos usuários de cateterismo urinário em cinco regiões de saúde: unidades básicas, unidades de saúde da família, ambulatórios, centros especializados, hospitais e unidades de pronto atendimento. Os dispositivos urinários dispensados foram o cateter intermitente e o de demora, junto a outros materiais necessários para o procedimento. Conclusão: O estudo revela a alta quantidade de cateteres urinários distribuídos; a atenção primária à saúde foi o serviço que direcionou o cuidado a partir da distribuição de materiais. Conclui-se que a construção de um mapa do cuidado oferecido possibilita a compreensão do cuidado ofertado e o desenvolvimento de demais estudos com estratégias de cuidado em rede aos usuários de cateterismo urinário.

Objetivo:avaliar a satisfação da pessoa com retenção urinária diante do uso de diferentes tipos de cateteres vesicais bem como comparar e analisar diferentes atributos desses após a sua utilização no cateterismo intermitente limpo (CIL). Método: estudo exploratório descritivo de corte transversal, desenvolvido em um ambulatório de referência do estado de São Paulo, no período de fevereiro a março de 2020. A amostra constituiu-se de 32 adultos cadastrados no serviço, e que adotavam o CIL como forma de esvaziamento vesical. A coleta de dados foi realizada por entrevista individual com a utilização de um questionário semiestruturado. Os dados foram analisados por estatística descritiva, inferencial e categorização das respostas para as questões abertas. Resultados: dos 32 usuários, 84% eram do sexo masculino, com média de idade de 54,16 anos, 81% realizavam o procedimento entre 1 a 5 anos, com frequência de 4 a 6 vezes ao dia (66%). A maioria dos entrevistados (73%) preferiu o cateter de poliuretano com revestimento hidrofílico, convergindo com o percentual de usuários satisfeitos com o mesmo cateter. Conclusão: a tecnologia com maior satisfação é o cateter de poliuretano com revestimento hidrofílico devido a diversos atributos, como lubrificação, firmeza, embalagem, deslizamento na uretra, manipulação, eliminação rápida da urina e conforto


Objective:to assess the satisfaction of the person with urinary retention in the face of the use of different types of urinary catheters, as well as to compare and analyze different attributes of these after their use in clean intermittent catheterization (CIC). Method: descriptive exploratory cross-sectional study, developed in a reference clinic in the state of São Paulo, from February to March 2020. The sample consisted of 32 adults registered in the service, and who adopted the CIC as a form of bladder emptying . Data collection was performed through individual interviews using a semi-structured questionnaire. Data were analyzed using descriptive, inferential statistics and categorization of responses to open questions. Results: of the 32 users, 84% were male, with a mean age of 54.16 years, 81% performed the procedure between 1 and 5 years, with a frequency of 4 to 6 times a day (66%). Most respondents (73%) preferred the hydrophilic-coated polyurethane catheter, converging with the percentage of users satisfied with the same catheter. Conclusion: the most satisfying technology is the hydrophilic-coated polyurethane catheter due to several attributes such as lubrication, firmness, packaging, urethral slip, handling, rapid urine clearance and comfort

La muestra se constituye de 32 adultos registrados en el servicio, y que adoptaban el CIL como forma de vaciamiento vesical. La recogida de datos fue realizada por entrevista individual con la utilización de un cuestionario semiestructurado. Los datos fueron analizados por estadística descriptiva, inferencial y categorización de las respuestas para las preguntas abiertas. Resultados: de los 32 usuarios, 84% eran del sexo masculino, con promedio de edad de 54,16 años, 81% realizaban el procedimiento entre 1 a 5 años, con frecuencia de 4 a 6 veces al día (66%). La mayoría de los entrevistados (73%) prefirió el catéter de poliuretano con revestimiento hidrófilo, convergiendo con el porcentual de usuarios satisfechos con el mismo catéter. Conclusión: la tecnología con mayor satisfacción es el catéter de poliuretano con revestimiento hidrófilo debido a diversos atributos, como lubricación, firmeza, embalaje, deslizamiento en la uretra, manipulación, eliminación rápida de la orina y comodidad

ABSTRACT

INTRODUCCIÓN: El piometra es una afección infrecuente, pero grave, que en general se diagnostica en mujeres posmenopáusicas. En adolescentes es sumamente raro, y si se acompaña de amenorrea primaria hay que tener en mente las anomalías congénitas. CASO CLÍNICO: Adolescente de 13 años, sin antecedentes personales de interés salvo amenorrea primaria, que acude con abdomen agudo y es intervenida por una peritonitis difusa causada por un piometra secundario a disgenesia (estenosis) cervical congénita. Se realizó dilatación cervical y se dejó una sonda vesical intrauterina para prevenir la reestenosis. CONCLUSIONES: Un diagnóstico precoz y un tratamiento conservador con dilatación cervical y colocación temporal de un catéter urinario son esenciales para un manejo seguro y efectivo de la estenosis cervical en adolescentes.


INTRODUCTION: Pyometra is an uncommon but serious condition that is generally diagnosed in postmenopausal women. In adolescents it is extremely rare; if accompanied by primary amenorrhea, consider congenital abnormalities. CASE REPORT: A 13-year-old adolescent, with no relevant personal history except primary amenorrhea, who presented with an acute abdomen and was operated on for diffuse peritonitis caused by pyometra secondary to congenital cervical dysgenesis (stenosis). Cervical dilation was performed and a urinary catheter was temporarily placed inside the uterus to prevent restenosis. CONLUSIONS: An early diagnosis and conservative treatment with cervical dilation and temporary placement of a urinary catheter are essential for the safe and effective management of cervical stenosis in adolescents.

ABSTRACT

RESUMEN El síndrome de la bolsa de orina púrpura es una entidad poco frecuente, con una prevalencia hasta de un 8% en pacientes con catéteres permanentes crónicos por más de 2 años. Esta condición se presenta en portadores de sonda vesical, siendo su principal signo clínico la aparición de orina de color púrpura, debida a la producción de sulfatasas y fosfatasas por enterobacterias. Se presenta un caso de una mujer de 63 años con diagnóstico de vejiga neurogénica, antecedente de enfermedad cerebrovascular y secuelas motoras debidas a neoplasia cerebral, quien consultó por presentar el color característico, con síntomas de infección del tracto urinario inferior. Se trató con nitrofurantoína y tuvo normalización del color urinario al quinto día de tratamiento. El diagnóstico de este síndrome implica un reto para el médico, y deben detectarse los factores de riesgo para proveer un adecuado manejo antimicrobiano y evitar el uso de recursos diagnósticos innecesarios. ABSTRACT Purple urine bag syndrome is a rare entity, with a prevalence of up to 8% in patients with chronic indwelling catheters for more than 2 years. This condition occurs in bladder catheter carriers, being the main clinical sign the appearance of purple urine, due to the production of sulphatases and phosphatases by Enterobacteriaceae. We present a clinical case of a 63-year-old woman with a diagnosis of neurogenic bladder and a history of cerebrovascular disease and motor sequelae due to brain neoplasia, who consulted for presenting that urine characteristic color, with symptoms of lower urinary tract infection. She was treated with nitrofurantoin and she had normalization of urinary color on the fifth day of treatment. The diagnosis of this syndrome implies a challenge for the physician, and risk factors must be detected to provide adequate antimicrobial management and avoid the use of unnecessary diagnostic resources. ABSTRACT

Objetivou-se no presente estudo comparar as técnicas de coleta de urina via sondagem uretral e cistocentese guiada por ultrassom, afim de verificar se o método de coleta pode influir nos resultados laboratoriais. Foram utilizados 12 cães machos, sem histórico de enfermidades, dos quais coletou-se cinco mililitros (mL) de urina via sondagem uretral e cinco mL via cistocentese guiada por ultrassom, ambas no mesmo momento. Posteriormente foi realizada a análise física (cor, odor, densidade, turbidez), química (urobilinogênio, glicose, corpos cetônicos, bilirrubina, proteína, nitrito, pH, sangue e leucócitos) e sedimentoscopia (avaliação de 10 campos de luz, objetiva de 40x). Cilindros urinários, cristais, corpúsculos gordurosos, espermatozoides, bactérias e células vesicais foram classificados qualitativamente como: ausentes (0), discretos (1), moderados (2) e intensos (3). Hemácias, leucócitos, e células de descamação foram quantificadas a partir da média dos campos analisados. As análises bioquímicas de microalbuminúria, creatinina e proteína total urinárias foram realizadas a partir do sobrenadante urinário, removido das amostras após centrifugação, e utilizados kits reagentes, conforme recomendação do fabricante, sendo a leitura em espectrofotômetro. Em todos os testes realizados os valores de p encontrados foram superiores 0,05 (p>0,05), excluindo-se a possibilidade de haver diferenças significativas dos resultados laboratoriais obtidos pelas duas formas de coleta.


The objective of this study was to compare two techniques of urine collection, urethral catheterization and ultrasound-guided cystocentesis, in order to verify if the collection method may influence the laboratory results. Twelve male dogs were used, with no history of diseases, of which five milliliters (mL) of urine were collected by urethral catheterization and five mL by both at the same time. Subsequently, the samples underwent physical analysis (color, smell, density andturbidity), chemical analysis (urobilinogen, glucose, ketone bodies, bilirubin, protein, nitrite, pH, blood and leukocytes) and sedimentoscopy (evaluation of 10 light fields, 40x objective). Urinary casts, fatty corpuscles, spermatozoa, bacteria and bladder epithelial cells were classified qualitatively as absent (0), discrete (1), moderate (2) and intense (3). Red blood cells, leukocytes and desquamation cells were quantified from the mean of the analyzed fields. The urine supernatants were obtained after centrifugation and were used for biochemical analyzes of microalbuminuria, urinary protein and creatinine. The reagent kits were used as recommended by the manufacturer and the samples were read by spectrophotometry. All tests presented p values higher than 0,05 (p>0,05), excluding the possibility of significant differences between the laboratory results of both forms of urine collection.

ABSTRACT Objective To evaluate the effectiveness of the Arco de Maguerez in teaching nursing students about the Bladder Catheter. Method Quasi-experimental study conducted at a public university in northeastern Brazil, with 29 students, in a single group, in the period of october 2018. Data were collected before and after using the Arco de Maguerez in educational intervention, using validated instruments, referring to knowledge and practice. IBM SPSS Statistics software version 24 was used for statistical analysis. The level of significance was set at 5% and the 95% confidence interval. Data analysis was performed using the McNemar and Mann-Whitney test. Results The median of correct answers for knowledge was 7, in the pre-test, and 9 in the post-test (p> 0.001). Regarding practice, in the pre-test the median of correct answers was 28 and, in the post-test, it went to 36, p> 0.001. The academics explained that the AM technology based on the problematization of a fictitious clinical case made learning about Bladder Catheterization Delay clear and feasible. Conclusion The Arco de Maguerez was effective in improving the knowledge and skill of nursing students regarding the Bladder Catheter.


RESUMEN Objetivo Evaluar la efectividad del Arco de Maguerez para enseñar a los estudiantes de enfermería sobre el cateter vesical. Método Estudio cuasiexperimental realizado en una universidad pública del noreste de Brasil, con 29 estudiantes, en un solo grupo, en el período de octubre de 2018. Los datos se recopilaron antes y después de usar el Arco de Maguerez en la intervención educativa, utilizando instrumentos validados, en referencia al conocimiento y la práctica. Para el análisis estadístico se utilizó el software IBM SPSS Statistics versión 24. El nivel de significancia se estableció en el 5% y el intervalo de confianza del 95%. El análisis de los datos se realizó mediante la prueba de McNemar y Mann-Whitney. Resultados La mediana de las respuestas correctas para el conocimiento fue 7, en la prueba previa y 9 en la prueba posterior (p> 0.001). En cuanto a la práctica, en la prueba previa la mediana de las respuestas correctas fue 28 y, en la prueba posterior, fue a 36, p> 0.001. Los académicos explicaron que la tecnología AM basada en la problematización de un caso clínico ficticio hizo que el aprendizaje sobre el Retardo de Cateterización de la Vejiga fuera claro y factible. Conclusión El Arco de Maguerez fue eficaz para mejorar el conocimiento y la habilidad de los estudiantes de enfermería con respecto al cateter vesical.


RESUMO Objetivo Avaliar a efetividade do Arco de Maguerez no ensino de acadêmicos de enfermagem sobre o Cateter Vesical de Demora. Método Estudo quase-experimental realizado em universidade pública no nordeste brasileiro, com 29 discentes, em grupo único, no período de outubro de 2018. Os dados foram coletados antes e depois da utilização do Arco de Maguerez em intervenção educativa, a partir de instrumentos validados, referentes ao conhecimento e prática. Foi utilizado o software IBM SPSS Statistics versão 24 para análise estatística. O nível de significância adotado foi de 5% e o intervalo de confiança de 95%. A análise dos dados ocorreu a partir do teste de McNemar e Mann-Whitney. Resultados A mediana de acertos do conhecimento foi de 7, no pré-teste, e 9 no pós-teste (p>0,001). Em relação à prática, no pré-teste a mediana de acertos foi de 28 e, no pós-teste, passou para 36, p>0,001. Os acadêmicos expuseram que a tecnologia do AM baseada na problematização de caso clínico fictício tornou o aprendizado sobre Cateterismo Vesical de Demora claro e factível. Conclusão O Arco de Maguerez foi efetivo para melhorar o conhecimento e a habilidade dos acadêmicos de enfermagem, referentes ao Cateterismo Vesical de Demora.

ABSTRACT

Objective To verify the relationship between catheter-related urinary tract infection(CAUTI)and stress hyperglycemia during catheter retention in stroke patients. Methods We used nosocomial infection monitoring system to track the status of CAUTI in stroke patients in a hospital.The study cohort was all the patients who received retention catheterization from January 2016 to March 2020.According to the nested case-control design,multivariate logistic regression analysis was performed to explore the relationship between stress hyperglycemia and CAUTI in stroke patients with indwelling catheter. Results A total of 322 cases of CAUTI and 644 cases of non-CAUTI were enrolled in this study.The length of stay in the case group was(20.68 ± 3.73)d,significantly longer than that[(13.00 ± 4.01)d]in the control group(t=29.473,P <0.001).Compared with non-stress hyperglycemia,stress hyperglycemia posed a higher risk of CAUTI in the stroke patients with indwelling catheter(OR=2.020,95% CI=1.447-2.821,P=0.000)and led to the higher incidence of CAUTI in one thousand days(P<0.001). Conclusion Stress hyperglycemia in the stroke patients with indwelling catheter can significantly increase the risk of CAUTI.

ABSTRACT

Resumen La infección del tracto urinario asociada al catéter urinario permanente (ITU/CUP) es un problema relevante en los centros de salud por su alta frecuencia. Objetivo: Identificar factores de riesgo de ITU en pacientes adultos con CUP. Material y Método: Estudio caso control efectuado entre los años 2010-2016 en el Hospital Militar de Santiago. Se realizó un análisis estadístico univariado y multivariado por medio del modelo de regresión logística binaria, con variables como edad, duración e indicación del dispositivo, algunas comorbilidades y la instalación del dispositivo luego de 15 días hospitalización. Resultado: Se obtuvo un total de 63 casos y 123 controles. Fueron variables predictoras de ITU/CUP la duración del CUP desde el séptimo día en adelante (OR 2,6 IC 1,4-4,9, p = 0,004) y la instalación del CUP con una estadía de hospitalización previa de 15 días y más (OR 7,8 IC 2,920,9 p = 0,000). No se encontró asociación con la edad mayor de 80 años, comorbilidades como diabetes y vejiga neurogénica e indicación de instalación. Conclusiones: Los resultados permiten focalizar las intervenciones, evaluando la necesidad real de indicación de CUP en pacientes con estadía hospitalaria previa de dos semanas y fomentar el retiro de CUP antes del séptimo día de uso.


Abstract Catheter-associated urinary tract infection (CAUTI) is a relevant problem in health centers because of its high frequency. Objective: To identify UTI risk factors in adult patients with urinary catheter. Material and Method: Control case study carried out between the years 2010-2016 at the Military Hospital of Santiago, Chile. Univariate and multivariate statistical analysis were performed using the binary logistic regression model, variables such as age, duration and indication of the device, some comorbidities and the installation of the device after 15 days of hospitalization. Result: A total of 63 cases and 123 controls were obtained. Predictive variables of CAUTI for the duration of the urinary catheter from the 7th day onwards (OR 2.6 IC 1.4-4.9, p = 0.004) and the installation of the urinary catheter with a previous hospital stay of 15 days and more (OR 7.8 CI 2.9-20.9 p = 0.000). No association was found in age over 80 years, comorbidities such as diabetes and neurogenic bladder and indication. Conclusions: The results permitted to focus the interventions, evaluating the real need for indication of CUP in patients with previous hospital statistics of 2 weeks and encouraging the withdrawal of CUP before the 7th day of use.

ABSTRACT Purpose: Continent urinary diversion (CUD) with the Mitrofanoff principle stands as an alternative to urethral catheterization by a route other than the urethra. The aim of the study was to determine self-perception of health-related quality of life (HRQoL), ease of catheterization and global and cosmetic outcomes in patient's dependent on Mitrofanoff catheterization. Materials and methods: Records of all patients who underwent CUD with the Mitrofanoff principle between 2012 to 2018 were reviewed. Data were collected and analysed retrospectively from medical charts. We assessed HRQoL with the EuroQol EQ-5D-3L questionnaire, cosmetic and global satisfaction with a questionnaire designed by the reconstructive urology board and ease of catheterization with a Likert questionnaire adapted from the Intermittent Catheterization Difficulty Questionnaire (ICDQ) validated in patients reliant on retrograde CIC. Results: A total of 25 patients requiring CUD with the Mitrofanoff principle between 2012 and 2018 were assessed, the group was composed mainly of: appendiceal conduits 18 patients (72%) and 7 ileal conduits (Yang-Monti) and three of those requiring Casale (Monti Spiral) and 1 a double Monti technique. Median follow-up was 57 months, median age was 30 years. Visual Analogue Scale (VAS) of the EQ-5D-3L reported a Global health score of 86.5%. Fifty nine percent of the patients had no pain or bleeding with catheterizations. Regarding global satisfaction and cosmetic perception 91% were satisfied with their CUD. Conclusions: CUD is associated with good HRQoL, global satisfaction, ease and painless catheterization, adequate self-perception of cosmetic outcomes and a low complication rate, remaining a safe and viable option.

Metodologia: Estudo de coorte, prospectivo, com amostragem por conveniência envolvendo 790 pacientes com idade superior a 12 anos, em uso de cateter durante hospitalização. Os dados coletados do prontuário e fichas de notificação de infecção hospitalar foram analisados pelo programa SPSS. Resultados: Contribuíram para o desenvolvimento da ITU-AC a permanência com o cateter urinário por período >20 dias (OR 26,5; p<0,001), ser cateterizado mais de uma vez (OR 8,92; p<0,001) e hospitalização >30 dias (OR 26,8; p<0,001). Pacientes que desenvolveram ITU-AC apresentaram chance maior de mortalidade (OR 2,7; p<0,001). Conclusão:Frequência da inserção do dispositivo urinário, períodos prolongados de hospitalização e de permanência com o cateter contribuíram para o desenvolvimento de ITU-AC, e as chances de mortalidade foram aumentadas entre pacientes com essa infecção. Patients who developed CR-UTI presented a greater chance of mortality (OR 2.7; p<0.001). Conclusion: Frequency of urinary device insertion, prolonged periods of hospitalization and of permanence with the catheter contributed to the development of CR-UTI, and the chances of mortality were increased among patients with this infection. ABSTRACT

Considerando a realidade conservacionista dos animais mantidos em cativeiro, em especial os pequenos felídeos silvestres, objetivou-se, com este estudo, descrever o método de coleta de sêmen por cateterismo uretral em Leopardus guttulus cativos, anestesiados com cetamina-dexmedetomidina. Inicialmente foram utilizados 13 animais para coleta de sêmen por cateterismo mediante o uso de diferentes doses de cetamina-dexmedetomidina. Após determinação da dose anestésica ideal para coleta de sêmen nessa espécie, cinco L. guttulus machos foram submetidos a coletas seriadas de sêmen pelo método do cateterismo. A dose ideal para coleta de sêmen foi de 0,008mg/kg de dexmedetomidina e 10mg/kg de cetamina. Os valores médios para volume e concentração foram de 35,9µL e 552,8x106sptz/mL. Com média de 71% de motilidade e 3,1 de vigor, 68% dos espermatozoides apresentaram vitalidade (integridade de membrana) e 77% integridade acrossomal. Sobre as patologias espermáticas, obteve-se uma média de 28% de espermatozoides com defeitos maiores, 6% com defeitos menores e 67% normais. As vantagens do método, como a facilidade e o baixo custo, fazem recomendar sua utilização em L. guttulus, pois foram apresentados bons resultados quanto à concentração espermática, à motilidade, ao vigor, à viabilidade espermática e à integridade acrossomal, sendo uma técnica promissora para utilização em felinos selvagens.(AU)


Considering the conservationist reality of animals kept in captivity, especially the small wild felids, this study aimed to describe the semen collection method using urethral catheterization in captive Leopardus guttulus, anesthetized with ketamine-dexmedetomidine. Initially, 13 animals were used for semen collection using catheterization with different ketamine-dexmedetomidine doses. After determination of the best anesthetic dose for semen collection in this species, five male L. guttulus were submitted to serial semen collections using the catheter method. The dose for semen collection was 0.008mg/kg dexmedetomidine and 10mg/kg ketamine. The mean values for volume and concentration were 35.9µL and 552.8x106sptz/mL, with a mean of 71% motility, 3.1 vigor, and 68% of spermatozoa presented vitality and 77% presented acrosomal integrity. Sperm pathologies obtained an average of 28% of spermatozoa with major defects, 6% of spermatozoa with minor defects and 67% of normal spermatozoa. The method advantages such as ease and low cost lead us to recommend the use in L. guttulus, since it presented good results regarding sperm concentration, motility, vigor, sperm viability and acrosomal integrity, being a promising technique for use in wild cats.(AU)

SUMMARY OBJECTIVES To prospectively investigate the alterations and normal ranges of ureteral jet dynamics after double-J-stent (DJS) removal in patients who underwent renal transplantation (RTx). METHODS Patients who underwent RTx were prospectively evaluated between November 2017 and June 2018. After RTx, Doppler ultrasonography (D-US) was performed on all patients after DJS removal. Renal artery resistive index (RA-Ri), renal pelvis anterior-posterior diameter (RP-APD), pelvicalyceal system dilation (PCSD), and ureteral jet flow dynamics (maximum and average velocity; JETmax and JETave) were measured by D-US. Also, patients' demographics, estimated glomerular filtration rate (eGFR) levels, and acute rejection were investigated in the study. Patients were assessed two different times by D-US, about 6 and 12 weeks after DJS removal, and the two different measurements were compared with the Wilcoxon test and Chi-square test. RESULTS A total of 25 patients were evaluated in the study. Nonobstructive PCSD rate (12% vs 8%), JETave (18.8 vs 12.9 cm/sec), and JETmax (29.2 vs 20 cm/sec) levels were significantly decreased (p values are 0.01, 0.010 and 0.014, respectively). In addition, monophasic and square pattern rates were significantly observed to increase over time (p=0.035); however, ureteral jet patterns were correlated between the two different D-US measurements (R=0.225, p=0.032). CONCLUSION After RTx, dilation rate and ureteral jet flow velocities were significantly decreased, and monophasic and square JETpattern rates were significantly increased over time. Ureteral jet dynamics can provide useful information about the follow-up of peristaltic activity in the pelvic-ureteric system.


RESUMO OBJETIVOS Investigar prospectivamente as alterações e as variações normais da dinâmica do jato ureteral após a remoção do J-stent duplo (DJS) em pacientes submetidos a transplante renal (RTx). MÉTODOS Pacientes submetidos a RTx foram avaliados prospectivamente entre novembro de 2017 e junho de 2018. Após o RTx, o D-US foi realizado em todos os pacientes após a remoção do DJS. Índice de resistência da artéria renal (RA-Ri), diâmetro ântero-posterior da pelve renal (AP-DPR), dilatação do sistema pelvicaliceal (PCSD) e dinâmica do jato ureteral (velocidade máxima e média; JETmax e JETave) foram medidos por D-US. Além disso, a demografia dos pacientes, os níveis estimados de taxa de filtração glomerular (eGFR) e a rejeição aguda foram investigados no estudo. Os pacientes foram avaliados em dois momentos diferentes pelo D-US, cerca de 6 e 12 semanas após a remoção do DJS, e as duas medidas diferentes foram comparadas com o teste de Wilcoxon e o teste do qui-quadrado. RESULTADOS Um total de 25 pacientes foi avaliado no estudo. Taxa de PCSD não obstrutiva (12% vs. 8%), JETave (18,8 vs. 12,9 cm/seg) e JETmax (29,2 vs. 20 cm/seg), os níveis foram significativamente diminuídos (valores de p são 0,01, 0,010 e 0,014, respectivamente). Além disso, as taxas de padrão monofásico e quadrado foram significativamente observadas para aumentar ao longo do tempo (p=0,035); no entanto, padrões de jato ureteral foram correlacionados entre as duas diferentes medidas D-US (R=0,225, p=0,032). CONCLUSÃO Após o RTx, a velocidade de dilatação e as velocidades de fluxo do jato ureteral foram significativamente diminuídas e as taxas de JET padrão monofásico e quadrado foram significativamente aumentadas ao longo do tempo. A dinâmica do jato ureteral pode fornecer informações úteis sobre o acompanhamento da atividade peristáltica no sistema pélvico-ureteral.

ABSTRACT Objective: Pyeloplasty is considered the gold standard treatment for ureteropelvic junction obstruction (UPJO). However, the failure rate of pyeloplasty is as high as 10% and repeat pyeloplasty is more difficult. This study aimed to evaluate the efficacy of balloon dilatation for failed pyeloplasty in children. Materials and Methods: Between 2011 and 2017, 15 patients, aged 6 months to 14 years, were treated with balloon dilation for restenosis of UPJO after a failed pyeloplasty. Ultrasound and intravenous urography were used to evaluate the primary outcome. Success was defined as the relief of symptoms and improvement of hydronephrosis, which was identified by ultrasound at the last follow-up. Results: All patients successfully completed the operation, 13 patients by retrograde approach and 2 patients by antegrade approach. Thirteen patients were followed for a median of 15 (4 to 57) months and 2 patients were lost to follow-up. Resolution of the hydronephrosis was observed in 5 cases. The anteroposterior diameter (APD) of the pelvis decreased by an average of 12.4 ± 14.4mm. Eight patients needed another surgery. The average postoperative hospital stay was 1.78 ± 1.4 days. Two patients experienced fever after balloon dilation. No other complications were found. Conclusions: Balloon dilatation surgery is safe for children, but it is not recommended for failed pyeloplasty in that group of patients, owing to the low success rate.

ABSTRACT

El objetivo de esta revisión integrativa fue identificar la utilidad de la cateterización urinaria intermitente o permanente en mujeres en labor de parto con parto vaginal y las posibles consecuencias en el postparto inmediato para mejorar la práctica clínica obstétrica. En la metodología se consideró las recomendaciones del Joanna Briggs Institute, partiendo de una pesquiza general para luego redactar una pregunta utilizando el formato PCC (Población, concepto y contexto); luego se estableció una estrategia de búsqueda de información en bases de datos, se llevó a cabo la selección y análisis crítico e interpretración de la evidencia encontrada. Identificados un total de 322 artículos, siendo removidos cinco por duplicación y excluídos 305 por título y resumen, quedando seleccionados 6. Como resultado se destacan cuatro temas los cuales fueron discutidos según la evidencia: 1. Cateterismo vesical en labor de parto; 2. Retención urinaria; 3. Distención vesical y labor de parto; 4. Infección urinaria y cateterismo vesical. Se concluye que enfermería obstétrica debe fomentar el vaciamiento vesical mínimo cada dos horas y estar haciendo valoraciones continuas e integrales durante todo el proceso de parto y postparto para detectar con prontitud cualquier desviación en la función correcta de la vejiga


The objective of this integrative review was to identify the usefulness of intermittent or permanent urinary catheterization in women in labor with vaginal delivery and the possible consequences in the immediate postpartum period to improve obstetric clinical practice. In the methodology the recommendations of the Joanna Briggs Institute were considered, starting from a general survey and then writing a question using the PCC format (Population, concept and context). Then a search strategy for information was established in databases, the selection and critical analysis and interpretation of the evidence found was carried out. Identified a total of 322 articles, being removed five by duplication and excluded 305 by title and summary, being selected 6. As a result four issues stand out which were discussed according to the evidence: 1. Bladder catheterization in labor; 2. Urinary retention; 3. Bladder distention and labor; 4. Urinary infection and bladder catheterization. It is concluded that obstetric nursing should encourage minimal bladder emptying every two hours and be making continuous and comprehensive assessments throughout the birth and postpartum process to detect any deviation in correct bladder function promptly

ABSTRACT

Objetivo: elaborar e validar material educativo escrito e ilustrativo para homens em uso de cateter urinário de demora no ambiente domiciliar. Material e Método: letramento em saúde, educação em saúde, modelos de validação de conteúdo diagnóstico de Enfermagem e Suitability Assessment of Educational Materials (SAM) foram referenciais teóricos e metodológicos utilizados. Resultados e discussão: realizaram-se a validação de conteúdo e aparência do material com base na avaliação de 30 peritos. Quanto ao conteúdo, todas as orientações foram validadas com escore final igual ou superior a 0,80. A aparência foi avaliada positivamente em todos os itens por 89,7% dos peritos. As sugestões contribuíram sobremaneira para melhor qualidade do material. Conclusão: considera-se que a validação de conteúdo e aparência com base nos referenciais utilizados pode contribuir como método de validação de materiais educativos em saúde.(AU)


Objective: to elaborate and validate a written and illustrative educational material aimed at men using home indwelling urinary catheters. Material and Method: health literacy, health education, nursing content validation models, and the Suitability Assessment of Material (SAM) were the theoretical and methodological references used. Results and discussion: the content and appearance of the material were validated based on the evaluation of 30 experts. Regarding the content, all guidelines were validated with a final score equal to or greater than 0.80. The appearance of the material was evaluated positively in all items by 89.7% of the experts. The suggestions have contributed greatly to increase the quality of the material. Conclusion: it is considered that the validation of content and appearance based on the references used can contribute as a validation method of educational materials in health.(AU)


Objetivo: elaborar y validar material educativo escrito e ilustrativo para hombres que usan catéter urinario de demora en el domicilio. Material y método: alfabetismo en salud, educación en salud, modelos de validación de contenido diagnóstico en enfermería y Suitability Assessment of Educational Materials (SAM) sirvieron como referentes teóricos. Resultados y discusión: se validó el contenido y la apariencia del material en base a la evaluación de 30 especialistas. Todas las orientaciones referentes al contenido recibieron puntuación final igual o superior a 0,80. El 89,7% de los especialistas evaluó positivamente todos los puntos referentes a la apariencia. Las sugerencias fueron de gran utilidad para mejorar la calidad del material. Conclusión: se considera que la validación de contenido y apariencia en base a los referentes empleados puede ayudar como método de validación de material educativo en salud.(AU)
